RESUMO

Organ transplantation is the most effective treatment for various types of end-stage diseases. To resolve the problem of donor shortage in organ transplantation, the possibility of xenotransplantation has been gradually explored by surgeons. Pig is one of the common donor sources for xenotransplantation. As a bridge between two species, the viruses carried by pig organs may be transmitted between species and cause the risk of zoonosis. Porcine endogenous retrovirus (PERV) is integrated into the genome, which is a category of retrovirus featuring cross-species transmission. In this article, the influencing factors of transmission characteristics of PERV, the transmission risk of PERV and its recombinant virus, and the detection and transmission risk assessment of PERV in xenotransplantation test were reviewed, aiming to provide reference for alleviating severe shortage of donor organs and driving the advancement of xenotransplantation technologies.

RESUMO

Objective: To investigate the risk factors of childhood systemic lupus erythematosus (SLE) with thyroid dysfunction and to explore the relationship between thyroid hormone and kidney injury of lupus nephritis (LN). Methods: In this retrospective study, 253 patients who were diagnosed with childhood SLE and hospitalized in the First Affiliated Hospital of Zhengzhou University from January 2019 to January 2021 were enrolled in the case group, and 70 healthy children were the control cases. The patients in the case group were divided into the normal thyroid group and the thyroid dysfunction group. Independent t-test, χ2 test, and Mann-Whitney U test were used for comparison between the groups, Logistic regression analysis was used for multivariate analysis, and Spearman correlation. Results: A total of 253 patients, there were 44 males and 209 females in the case group, and the age of onset was 14 (12, 16) years; a total of 70 patients, 24 males and 46 females were in the control group, and the age of onset was 13 (10, 13) years. The incidence of thyroid dysfunction in the case group was higher than that in the control group (48.2% (122/253) vs. 8.6% (6/70), χ²=36.03, P<0.05). Of the 131 patients, there were 17 males and 114 females in the normal thyroid group, and the age of onset was 14 (12, 16) years. Of the 122 patients in the thyroid dysfunction group, 28 males and 94 females were in the thyroid dysfunction group, and the age of onset was 14 (12, 16) years. Of the 122 had thyroid dysfunction, including 51 cases (41.8%) with euthyroid sick syndrome, 25 cases (20.5%) with subclinical hypothyroidism, 18 cases (14.8%) patients with sub-hyperthyroidism, 12 cases (9.8%) with hypothyroidism, 10 cases (8.2%) with Hashimoto's thyroiditis, 4 cases (3.3%) with hyperthyroidism, and 2 cases (1.6%) with Graves disease. Compared to patients with normal thyroid function, the serum level of triglyceride, total cholesterol, urine white blood cell, urine red blood cell, 24 h urine protein, D-dimer, and fibrinogen, ferritin and systemic lupus erythematosus disease activity Index-2000 (SLEDAI-2K) score were higher in patients with thyroid dysfunction (Z=3.07, 3.07, 2.48, 3.16, 2.40, 3.99, 2.68, 2.55, 2.80, all P<0.05), while the serum level of free thyroxine and C3 were lower in thyroid disfunction patients (10.6 (9.1, 12.7) vs. 11.3 (10.0, 12.9) pmol/L, and 0.46 (0.27, 0.74) vs. 0.57 (0.37, 0.82) g/L, Z=2.18, 2.42, both P<0.05). The higher level of triglyceride and D-dimer were the independent risk factors for childhood SLE with thyroid dysfunction (OR=1.40 and 1.35, 95%CI 1.03-1.89 and 1.00-1.81, respectively, both P<0.05). There were 161 patients with LN in the case group, all of which were conducted with renal biopsies, including 11 cases (6.8%) with types Ⅰ LN, 11 cases (6.8%) with typesⅡLN, 31 cases (19.3%) with types Ⅲ LN, 92 cases (57.1%) with types Ⅳ LN, and 16 cases (9.9%) with types Ⅴ LN. There were significant differences in the level of free triiodothyronine and thyroid stimulating hormone among different types of kidney pathology (both P<0.05); compared with types I LN, the serum level of free triiodothyronine was lower in types Ⅳ LN (3.4 (2.8, 3.9) vs. 4.3 (3.7, 5.5) pmol/L, Z=3.75, P<0.05). The serum level of free triiodothyronine was negatively correlated with the acute activity index score of lupus nephritis (r=-0.228, P<0.05), while the serum level of thyroid stimulating hormone was positively correlated with the renal pathological acute activity index score of lupus nephritis (r=0.257, P<0.05). Conclusions: There is a high incidence of thyroid dysfunction in childhood SLE patients. The higher SLEDAI and more severe renal damage were found in SLE patients with thyroid dysfunction compared to these with normal thyroid functions. The risk factors of childhood SLE with thyroid dysfunction are the higher level of triglyceride and D-dimer. The serum level of thyroid hormone is possibly related to the kidney injury of LN.

RESUMO

Posterior capsule opacification (PCO), a common complication after cataract surgery, impacts a patient's long-term visual quality to various degrees. Although a neodymium:yttrium aluminum garnet (Nd:YAG) laser posterior capsulotomy is a very effective treatment, it may lead to a serial of complications. Accordingly, the search for simple, safe, and effective methods to prevent PCO has received widespread attention. Various researchers are committed to the interdisciplinary collaboration between medicine and engineering fields, such as functionalizing the surface of the intraocular lens (IOL) via supercritical fluid impregnation, coating the surface of the IOL, high-concentration drug immersion, and application of a drug delivery system, to effectively reduce the incidence and severity of PCO.

RESUMO

ObjectiveTo observe the effects of modified Chaihu Shugansan(CHSG) and its disassembled formulas on angiotensin-converting enzyme 2 (ACE2)-angiotensin (Ⅰ-Ⅶ) [Ang (Ⅰ-Ⅶ)]-mitochondrial assembly receptor (MasR) axis in hyperlipidemic rats with myocardial ischemia and depression, and to explore the underlying mechanism of its prevention and treatment of myocardial ischemia and depression. MethodA total of 108 male SD rats were randomly divided into a normal group, a model group, a modified CHSG group (11.7 g·kg-1), a Quyu Huatan disassembled formula group (4.05 g·kg-1), a Shugan Xingqi disassembled formula group (3.15 g·kg-1), a Jianpi Yangxue disassembled formula group (4.5 g·kg-1), a fluoxetine group (0.001 8 g·kg-1), a trimetazidine group (0.005 4 g·kg-1), and a simvastatin group (0.001 8 g·kg-1), with 12 rats in each group. The hyperlipidemia model with myocardial ischemia and depression was induced with a high-fat diet combined with injection of isoproterenol (ISO) and chronic unpredictable mild stress (CUMS) in rats in the model group and groups with drug intervention for eight weeks. The rats in each group with drug intervention were treated correspondingly by gavage from the first day of modeling, while those in the normal group and the model group received the same amount of normal saline. The behavioral changes of rats in each group were observed by open field test and forced swimming test. Left ventricular fractional shortening (LVFS) and left ventricular ejection fraction (LVEF) were measured by echocardiography. The serum levels of total cholesterol (TC), triglyceride (TG), low-density lipoprotein cholesterol (LDL-C), and high-density lipoprotein cholesterol (HDL-C) were detected by the enzyme-labeled apparatus. Hematoxylin-eosin (HE) staining was used to observe the histomorphological changes of the heart. The serum levels of angiotensin Ⅱ (AngⅡ), ACE2, and Ang(Ⅰ-Ⅶ) were detected by enzyme-linked immunosorbent assay (ELISA). The protein and mRNA expression of ACE2 and MasR in the hippocampus and the heart was detected by real-time quantitative polymerase chain reaction (Real-time PCR) and Western blot. ResultCompared with the normal group, the model group showed reduced movement time, distance, and average speed in the central area of the open field (P<0.01), prolonged immobility time of rats in the forced swimming test (P<0.01), decreased LVFS and LVEF (P<0.01), inflammatory exudation and disorderly arranged fiber in heart tissues, elevated serum levels of TC, LDL-C, AngⅡ, ACE2 and Ang(Ⅰ-Ⅶ), diminished HDL-C (P<0.01), dwindled mRNA and protein expression of ACE2 in the hippocampus and the heart and MasR in the hippocampus, and up-regulated mRNA and protein expression of MasR in the heart (P<0.01). Compared with the model group, the modified CHSG group displayed increased movement time, distance, and average speed in the center area of the open field (P<0.01), shortened immobility time in the forced swimming test (P<0.01), increased LVFS and LVEF (P<0.01), relieved heart injury, reduced serum levels of TC, LDL-C, AngⅡ, ACE2, and Ang(Ⅰ-Ⅶ), elevated level of HDL-C (P<0.01), up-regulated mRNA and protein expression of ACE2 in the hippocampus and the heart and MasR in the hippocampus, and down-regulated mRNA and protein expression of MasR in the heart (P<0.01). Each disassembled formula could improve the above indexes to a certain extent (P<0.05, P<0.01), but the effect of the whole formula was optimal. ConclusionThe modified CHSG and its disassembled formulas have the effects of resisting depression, improving myocardial injury, and reducing blood lipid. Due to the synergistic effects of stasis-resolving/phlegm-eliminating drugs, liver-smoothing/Qi-moving drugs, and spleen-tonifying/blood-nourishing drugs in the formula, the modified CHSG is superior to each disassembled formula in efficacy. Its mechanism may be related to the activation of the ACE2-Ang (Ⅰ-Ⅶ)-MasR axis.

RESUMO

ObjectiveTo investigate inhibitory effect of extracts from Veronica peregrina (EVP) on the osteoclastic bone metastasis induced by breast cancer cells. MethodBone metastasis model was established by injection of MDA-MB-231 cells, a human breast cancer cell line, into the left ventricle of BALB/c nude mice. The expression of human cytokeratin-19 (Ck-19) gene in mouse bone marrow was determined by nested polymerase chain reaction(PCR) to assess the bone metastasis of MDA-MB-231 cells. To assess the effects of EVP on the activation of bone marrow macrophages (BMMs), we counted the multinuclear cells and measured the secretion of Cathepsin K. Western blot was adopted to assess the effects of EVP on receptor activator of nuclear factor-κB (RANK), Runt-related transcription factor 2 ( Runx2 ), phosphorylated Runx2 (p-Runx2), and matrix metalloproteinase-9 (MMP-9) in BMMs. Gelatin zymography was employed to determine the activities of matrix metalloproteinases (MMPs). ResultCompared with that in the blank group, Ck-19 expression was down-regulated in EVP groups (P<0.05). The multinucleated cells increased when the BMMs were induced by soluble receptor activator of nuclear factor-κB ligand (sRANKL), which was inhibited by EVP (P<0.05). The level of cathepsin K in the supernatant of sRANKL group increased compared with that of the blank group, while EVP groups had lower cathepsin K levels than sRANKL group (P<0.05). Compared with the blank group, the sRANKL group showed up-regulated RANK expression, Runx2 phosphorylation, and MMP-9 expression (P<0.05), while the expression levels of RANK, p-Runx2, and MMP-9 were down-regulated when the cells were incubated with EVP (P<0.05). Furthermore, exposure of BMMs to sRANKL resulted in an increase in gelatin hydrolyzation compared with the blank group (P<0.01), which, however, was reversed in EVP groups (P<0.05). ConclusionEVP significantly inhibits bone marrow metastasis of MDA-MB-231 cells, which may be associated with the suppression of osteoclast activation by inhibiting Runx2 phosphorylation.

RESUMO

【Objective】 To detect the piperacillin and amoxicillin antibodies in suspicious blood samples from pre-transfusion compatibility tests in Wuxi and analyze the general characteristics of them, so as to eliminate the interference of drug-induced antibodies with compatibility tests and provide reference for safe and effective blood transfusion, 【Methods】 Drug-sensitized RBCs and low-ion anti-globulin microcolumn gels were used to detect piperacillin and amoxicillin antibodies in 128 plasma samples which were initially undetermined in pre-transfusion compatibility tests. Data were analyzed by Chi-square test or fisher′s exact test. P<0.05 was statistically significant. 【Results】 Among these 128 undetermined samples, including 31 cases of type A, 48 type B, 14 type AB and 35 type O, the overall positive rate of piperacillin and amoxicillin antibodies was 28.9%(37/128), in which the positive rates of piperacillin and amoxicillin antibodies were 20.3%(26/128) and 8.6%(11/128), respectively. The difference between these two drug-induced antibodies was significant(P<0.05). Further analysis showed that the piperacillin antibodies in patients over 50 years old was 25.3%(24/95), while under 50 years old was 6.1%(2/33)(P<0.05). In contrast, the amoxicillin antibodies in patients over 50 years old was 5.3%(5/95), while under 50 years old was 18.2%(6/33), with statistically significant differences between each other(P<0.05). 【Conclusions】 In patients with suspicious antibodies in pre-transfusion detection, except for the allotype antibodies, drug-induced antibodies should be more considered in combination with medication history to better ensure the safety and effectiveness of blood transfusion.

RESUMO

Objective:To investigate the inhibitory effects of Danshen injection against ovarian cancer cell proliferation induced by the interaction between platelets and cancer cells. Method:The induction of platelets on SKOV3 growth in vitro and the inhibitory effect of Danshen injection at 12,24,and 48 g·L-1 were observed by 3-(4,5-Dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide (MTT) and cell colony formation assays. The content of transforming growth factor-β1 (TGF-β1) in the platelet-tumor cell interaction system and platelet supernatant and the effect of Danshen injection on TGF-β1 secretion were detected by enzyme-linked immunosorbent assay (ELISA). The influences of tumor cell culture supernatant on platelet aggregation and secretion and the inhibitory effect of Danshen injection were determined by microplate assay and ELISA. The effects of Danshen injection on platelet nuclear factor kappa B (NF-κB) signaling pathway were assayed by Western Blot. Result:Compared with the blank group, the platelet induction group exhibited significantly elevated absorbance at A570 (P<0.01), while the absorbance at A570 in the platelet + Danshen injection group was significantly lower than that in the platelet induction group (P<0.01). The comparison with the Danshen injection group revealed that the cell proliferation inhibitory rate in the platelet + Danshen injection group at the same dose was more significant (P<0.01). The number of colonies in the platelet induction group was obviously increased in contrast to that in the blank group(P<0.05), while the number of colonies in the platelet + Danshen injection group was significantly lower than that in the platelet induction group(P<0.05,P<0.01). As demonstrated by comparison with the blank group, TGF-β1 content in the supernatant of the platelet induction group rose remarkably(P<0.01), whereas that in the platelet + Danshen injection group declined(P<0.01). Compared with the Danshen injection (24 g·L-1) group, the platelet + Danshen injection group displayed more obvious inhibition(P<0.01). Compared with the blank group, Danshen injection significantly reduced the TGF-β1 content in platelet supernatant(P<0.05,P<0.01). There was no significant change in the content of TGF-β1 in SKOV3 supernatant treated with Danshen injection. The platelet aggregation, thromboxane A2(TXB2), and serotonin (5-HT) secretion in the SKOV3 cell supernatant induction group were significantly increased as compared with those in the blank group (P<0.01), while such indexes in the cell supernatant induction + Danshen injection group were obviously decreased (P<0.01). Compared with the Danshen injection (24 g·L-1) group, the cell supernatant induction + Danshen injection group displayed more obvious inhibition at the same dose(P<0.01). Compared with the blank group, the platelet induction group exhibited obviously up-regulated phosphorylated TGF-β-activated kinase-1 (TAK-1) and NF-κB, but down-regulated phosphorylated inhibitory protein of NF-κB (IκB)(P<0.01), which however were significantly reversed in the platelet + Danshen injection group(P<0.01). Conclusion:Danshen injection affect the proliferation of SKOV3 cells by inhibiting their interaction with platelets, which may be related to the inhibited secretion of TGF-β1.

RESUMO

Objective: To compare the recovery and quality of life of patients with oral and oropharyngeal tumors treated with three kinds of free soft tissue flaps. Methods: The clinical data of 103 patients, including 66 males and 37 females, aged 26-74 years, who underwent primary repair of defects after resection of oral and oropharyngeal tumors in Sichuan Tumor Hospital from July 2014 to August 2020 were analyzed. Anterolateral thigh flap (ALTF) was used in 43 patients, radial forearm free flap (RFFF) in 45 patients, and lateral arm free flap (LAFF) in 15 patients. Postoperative qualities of life of patients were evaluated by the university of Washington quality of life questionnaire and oral health impact scale (HIP-14 Chinese edition). SPSS 23.0 software was used for statistical analysis. Results: The T staging of RFFF or LAFF group was significantly lower than that of ALTF group (P<0.05). There was no significant difference in mean flap areas between ALTF group ((55.87±27.38) cm2) and LAFF group ((49.93±19.44) cm2), while RFFF group had smaller mean flap area ((33.18±6.05) cm2) than ALTF group (t=5.311, P<0.001) and LAFF group (t=3.284, P=0.005). In terms of oral functions including swallowing, mastication, taste and spitmouth, there were no significant differences between LAFF group and RFFF group (P>0.05), but both groups had better oral functions than ALTF group (P<0.05). There was no significant difference in appearance scores between LAFF group (75(75, 75)) and ALTF group (75(75,75) vs.75(75,75),Z=-1.532, P=0.126), and both groups had higher scores than RFFF group (50(50, 75),Z values were -3.447 and -3.005 respectively, P<0.05). RFFF group had higher speech score (100(67, 100)) than LAFF group (67(50, 76),Z=-2.480, P<0.05) and ALTF group (67(33, 67),Z=-5.414, P<0.05). ALTF group had lower mean score of quality of life than RFFF group [72(56,77) vs.79(69, 89),Z=-3.070, P<0.05), but there was no statistical difference in the mean scores of qualities of life between ALTF group and LAFF group (Z=1.754, P=0.079). According to the evaluation of oral health impact scale (HIP-14 Chinese version) 1 year after surgery, individual item scores and the average score of all items in ALTF group were lower than those in RFFF and LAFF groups (P<0.05), with no significant difference between RFFF group and LAFF group (P>0.05). Conclusions: RFFF has unique advantages for small tissue defects, while ALTF is suitable for large tissue defects, such as buccal penetrating defect, whole tongue and near whole tongue defect, and LAFF is a compromise choice between ALTF and RFFF. ALTF is inferior to RFFF and LAFF in oral functional reconstruction, including swallowing, chewing, taste and spittle. ALTF and LAFF are superior to RFFF in postoperative appearance.

RESUMO

OBJECTIVE: To test and analyze the vibration transmissivity of three common kinds of protective gloves, and to determine their actual protecting effect. METHODS: Three kinds of protective gloves were selected as test materials, and five healthy volunteers were selected as subjects using typical sampling methods. According to the ISO 10819:2013 Mechanical Vibration and Shock--Hand-arm Vibration--Measurement and Evaluation of the Vibration Transmissibility of Gloves at the Palm of the Hand(hereinafter referred to as ISO 10819:2013), investigation and measurement of basic parameters of protective gloves. The gloved vibration transmission(GVT) test system was used to test the vibration transmission of different protective gloves worn by the subjects, and the spectrum analysis of vibration reduction characteristics was carried out. RESULTS: The thickness of damping material of all the three kinds of protective gloves met the standard requirements. The mean value for the corrected frequency-weighted glove vibration transmissibility values at 25.0-200.0 Hz were 0.91, 0.75 and 0.94, and 1.05, 0.85 and 1.10 at 200.0-1 250.0 Hz, for the protective gloves A, B and C, respectively. The results of anti-vibration characteristic spectrum analysis show that there has no vibration reduction effect at the frequency of 25.0-200.0 Hz when wearing these three kinds of gloves. Wearing gloves A and B on part of the 200.0-1 250.0 Hz frequency vibration has an amplifying effect. Wearing glove C has a partial vibration reduction effect at the 200.0-1 250.0 Hz frequency, and the vibration reduction efficiency is the highest at the frequency of 1 250.0 Hz.CONCLUSION: According to the ISO 10819:2013 standard, the mean value of GVT measured when wearing the three kinds of protective gloves does not meet the relevant requirements, and the anti-vibration effect is poor. These gloves cannot be defined as anti-vibration gloves.

RESUMO

Objective@#: Covered stenting is an optional strategy for traumatic carotid pseudoaneurysm, especially in malignant conditions of potential rupture, but the long-term outcomes are not clear. Our aim was to determine if covered stenting is an effective option for traumatic carotid pseudoaneurysm with promising long-term outcomes. @*Methods@#: Self-expanding Viabahn and balloon-expandable Willis covered stents were separately implanted for extra- and intracranial traumatic carotid pseudoaneurysm. The covered stent was placed across the distal and proximal pseudoaneurysm leakage under roadmap guidance. Procedural success was defined as technical success (complete exclusion of the pseudoaneurysm and patency of the parent artery) without a primary end point (any stroke or death within 30 days after the procedure). Longterm outcomes were evaluated as ischemic stroke in the territory of the qualifying artery by clinical follow-up through outpatient or telephone consultation and as the exclusion of the pseudoaneurysm and patency of the parent artery by imaging follow-up through angiography. @*Results@#: Five patients with traumatic carotid pseudoaneurysm who underwent covered stenting were enrolled. The procedural success rate was 100%. No ischemic stroke in the territory of the qualifying artery was recorded in any of the five patients during a mean clinical follow-up of 44±16 months. Complete exclusion of the pseudoaneurysm and patency of the parent artery were maintained in all five patients during a mean imaging follow-up of 39±16 months. @*Conclusion@#: Satisfactory procedural and long-term outcomes were obtained, suggesting that covered stenting is an effective option for traumatic carotid pseudoaneurysm.

RESUMO

Objective:To compare the safety and immunogenicity of Sabin strain-based inactivated poliovirus vaccine (sIPV) and the liquid form of typeⅠ+ Ⅲ bivalent oral poliovirus vaccine (bOPV) administered to infants aged ≥2 months in different schedules. Methods:A randomized, blinded, single-center, parallel-group controlled trial was conducted in Hangzhou from 2017 to 2018. Healthy infants aged ≥2 months were enrolled and randomized to receive the vaccines on a schedule of 2, 3, 4 months. Group 1sIPV+ 2bOPV was given one dose of sIPV and two doses of bOPV; group 2sIPV+ 1bOPV was administrated two doses of sIPV and one dose of bOPV; group 3sIPV received three doses of sIPV. Adverse events (AEs) following vaccination were recorded. Blood samples were collected from the subjects (excluding the quitters or subjects against the trial plan) 28-35 d after the full-course immunization. A microneutralization assay was performed to detect the geometric mean titers (GMTs) of neutralizing antibodies against polio virus of Ⅰ, Ⅱ and Ⅲ types. The seroconversion rates of neutralizing antibodies were also calculated. Results:The overall incidence of AEs following vaccination was 3.57% in 1sIPV+ 2bOPV group, 3.61% in 2sIPV+ 1bOPV group and 1.19% in 3sIPV group (χ 2=1.190, P=0.552) and no severe AEs were reported. The antibody seroconversion rates in 1sIPV+ 2bOPV, 2sIPV+ 1bOPV and 3sIPV groups were respectively 100% (84/84), 100% (83/83) and 100% (84/84) against type Ⅰ poliovirus, 81% (68/84), 96% (80/83) and 99% (83/84) against type Ⅱ poliovirus(χ 2=21.469, P<0.001), and 100% (84/84), 100% (83/84) and 100% (84/84) against type Ⅲ poliovirus. In 1sIPV+ 2bOPV, 2sIPV+ 1bOPV and 3sIPV groups, the GMTs of antibody were 1 024.00, 1 015.48 and 982.61 against type Ⅰ poliovirus ( F=2.742, P=0.066), 16.81, 107.94 and 218.85 against type Ⅱ poliovirus ( F=33.570, P<0.001), and 990.75, 990.36 and 613.92 against type Ⅲ poliovirus ( F=37.886, P<0.001). Conclusions:sIPV and bOPV administered in different schedules showed good safety and immunogenicity in infants aged≥2 months. The GMT and the seroconversion rate of neutralizing antibody against type Ⅱ poliovirus after vaccination were higher in 2sIPV+ 1bOPV and 3sIPV group than in 1sIPV+ 2bOPV group. Higher GMT of neutralizing antibody against type Ⅲ poliovirus was induced in 1sIPV+ 2bOPV and 2sIPV+ 1bOPV groups than in 3sIPV group.

RESUMO

Objective:To investigate the effect of stimulation of the central thalamus (CT-DBS) on awakening from coma after traumatic brain injury and the mechanisms involved.Methods:Forty-five healthy, adult Wistar rats were randomly assigned to either a blank control group, a sham stimulation or a stimulation group, each of 15. CT-DBS was applied in the stimulation group. Behavioral changes were observed, and the content of α1-adrenergic receptor (α1R) in the prefrontal cortex (PFC) was measured using immunofluorescence and western blotting. Results:The varus reflex was found in all 15 rats in the stimulation group, but in only 8 among the sham stimulation group. The average α1R content in the PFC was significantly higher in the stimulation group than in the sham stimulation group.Conclusions:CT-DBS can improve the level of consciousness after traumatic brain injury and promote awakening, at least in rats. The mechanisms may be related to the up-regulation of α1R expression in the PFC.

RESUMO

OBJECTIVE@#To study the clinical effect and safety of tacrolimus (TAC) combined with glucocorticoid (GC) versus mycophenolate mofetil (MMF) combined with GC in the treatment of primary IgA nephropathy (IgAN) in children.@*METHODS@#A retrospective analysis was performed for the clinical data of children with primary IgAN confirmed by renal pathology between January 2012 and December 2017. These children were divided into TAC group and MMF group according to the treatment regimen. Their clinical data before treatment and at 1, 3, and 6 months of treatment were collected, and the remission status of IgAN and adverse reactions were compared between the two groups.@*RESULTS@#A total of 43 children who met the inclusion criteria were enrolled, with 15 children in the TAC group and 28 children in the MMF group. At 1 month of treatment, there was no significant difference in the remission status between the two groups (P>0.05). At 3 and 6 months of treatment, the TAC group had a significantly better remission status than the MMF group (P0.05), but fungal infection was observed in one child from the TAC group.@*CONCLUSIONS@#TAC combined with GC can effectively reduce urinary protein in children with primary IgAN, and it has a better short-term clinical effect than MMF combined with GC, with good safety.

RESUMO

BACKGROUND@#To investigate the effectiveness of topical application of 4% formaldehyde as a minimally invasive treatment of rectal bleeding due to chronic radiation proctitis (CRP) under direct vision of electronic colonoscope.@*METHODS@#The clinical data of 13 CRP patients complicated with ≥ grade II bleeding admitted to our hospital between January 2003 and December 2018 were retrospectively analyzed. Under the guidance of electronic colonoscope, 4% formaldehyde combined with 5-aminosalicylic acid (5-ASA) suppositories was topically applied. Patients were followed up for two months after treatment, and the therapeutic effectiveness was observed and analyzed.@*RESULTS@#The rectal bleeding due to CRP was markedly reduced after topical application of 4% formaldehyde under colonoscope in all 13 patients. The bleeding stopped after one treatment session in 11 patients and after the second session in 2 patients. 5-ASA was also applied along with the use of 4% formaldehyde. The therapeutic effectiveness was satisfactory during the 1- and 2-month follow-up period.@*CONCLUSION@#Topical application of 4% formaldehyde under the direct vision of colonoscope as a minimally invasive treatment for CRB-induced bleeding is a simple, effective, affordable, and repeatable technique without obvious complications, which deserves further exploration and promotion.

RESUMO

Objective:To observe the effects of sling exercise therapy on muscle strength of pelvic floor in patients with postpartum stress urinary incontinence. Methods:From February, 2018 to February, 2019, 90 patients diagnosed as postpartum stress urinary incontinence 42 days after delivery were randomly divided into control group (n = 45) and observation group (n = 45). The control group received Kegel training, and the observation group received sling exercise therapy, for a month. The electromyogram of pelvic floor was measured before and after treatment, and the clinical efficacy was evaluated at the end of the treatment. Results:The electromyogram of pelvic floor increased in both groups after treatment (t > 3.879, P < 0.01), and increased more in the observation group than in the control group (P < 0.05). The rate of improvement was 93.33% in the observation group, which was more than 75.56% in the control group (χ2 = 13.83, P < 0.05). Conclusion:Sling exercise therapy can more effectively improve the muscle strength of pelvic floor for patients with postpartum stress urinary incontinence, and relieve the symptoms of overfall.

RESUMO

Objective@#To review and summarize recent update on preoperative diagnostic criteria, treatment and postoperative follow-up for medullary thyroid carcinoma.@*Methods@#The relevant literatures and guidelines about medullary thyroid carcinoma were analyzed and summarized.@*Results@#In the early stages of the disease radical surgery still dominated. Ultrasound results suggested that prophylactic lateral neck dissection was required for patients with high risk factors or high levels of carcinoembryonic antigen and calcitonin need prophylactic. Early hereditary medullary carcinoma could receive prophylactic thyroidectomy based on RET gene test results. Advanced progressive medullary thyroid carcinoma could be treated with palliative surgery, molecular targeted drugs and chemotherapy.@*Conclusions@#The prognosis of medullary thyroid carcinoma is poor and lymph node metastasis is easy to occur early. The extent of initial operation should be enough. Locally advanced or distant metastatic medullary thyroid carcinoma can be treated with palliative surgery, molecular targeted drugs and chemotherapy.

RESUMO

Extrathyroidal extension of thyroid cancer has been an important adverse factor affecting the prognosis of patients. According to the latest NCCN (National Comprehensive Cancer Network) guidelines, extrathyroidal extension is the surgical guide fortotal thyroidectomy in newly diagnosed patients, and its incidence in differentiated thyroid cancer is 5%-34%, belonging to T3-T4 stage. In the eighth edition of thyroid cancer AJCC staging, the T3 stage was first divided into T3a (tumor>4 cm and limited to the thyroid) and T3b (gross extrathyroidal extension invading only strap muscles from a tumor of any size), and the "minimal extrathyroidal extension(tumor invasion intoperithyroidal soft tissue or strap muscle invasion)"of the seventh edition was removed from the T stage and changed to the gross extrathyroidal extension invading only strap muscles, but there is still much controversy. It can be seen that different degrees of "extrathyroidal extension" have significant differences in the survival and prognosis of thyroid cancer. This article reviews the latest research progress of extrathyroidal extension, and discusses the significance and clinical research progress of it.

RESUMO

@#【Abstract】 【Objective】To investigate the differential expression of long non-coding RNA-1708(lncRNA-1708)in osteogenic differentiation of human bone marrow mesenchymal stem cells(hBMSC)and its effect on osteogenic differentiation of hBMSC.【Methods】Purchased 3 groups of hBMSC from different sources and cultured in vitro. qRT-PCR was used to detect the expression of lncRNA-1708 after osteogenic differentiation of three groups of hBMSC,and the relationship between lncRNA-1708 and hBMSC osteogenic differentiation was analyzed. LncRNA-1708 overexpressing lentivirus and lncRNA-1708 interferenced plasmid were transfected respectively,to obtain stable hBMSC cell line. After 14-day osteogenic differentiation on transfected hBMSC,RT-PCR was used to detect the expressions of runt-related transcription factor 2(RUNX2)and alkaline phosphatase(ALP)mRNA,and alkaline phosphatase staining was made.【Results】The expression of lnc-1708 decreased after osteogenic differentiation of hBMSC for 7 d(P < 0. 001).Two cell lines, which respectively express high lncRNA-1708 and low lncRNA-1708,were constructed successfully. In lnc-1708-overexpressed BMSC,the mRNA levels of osteogenic markers RUNX2 and ALP were both significantly down-regulated(0.46±0.03 vs.1.00±0.02,0.15±0.07 vs. 1.02±0.28,P < 0.01). On the contrary,in the lnc-1708-silencing BMSC,the expressions of RUNX2 and ALP mRNA level were significantly up-regulated(1.62±0.18 vs. 1.00±0.04,1.58±0.11 vs. 1.01±0.18,P < 0.01).【Conclusion】LncRNA-1708 may have an inhibitory effect on osteogenic differentiation of hBMSC.
